- Shotgun/Firearm Form, certificate - £120
- Holiday cancellation letter - £60
- Sickness/Benefit letter - £60
- Medication for travel letter - £60
- Letter confirming address - £60
- Gym membership/sports event letter/form - £60
- Medical report – HGV, taxi, adoption, fostering etc - £150
- Power of Attorney forms - £150
- Bus pass form - £60
- Occupational Health form - £60
- Adoption/surrogacy form etc - £60
- Other reports – Insurance, ill health etc - £150
Payment is payable on collection of the completed documents – cash or cheque only.
Please be aware that the doctors can only comment on facts as recorded in the patient notes. Fees will NOT be refunded.
The surgery will not be able to witness form signatories or approval ID such as passport. We do not provide ‘Fit to’ letters – fly, drive, etc.
We reserve the right to not offer a service for extreme sports or activities. GP discretion/decision is final upon request.
Please allow 10 working days for completion of all requests.
Fees are chargeable as preparation of private letters, reports and referrals are not covered by the NHS and the doctors must complete such work in their own time and as such, there is a fee payable. A GPs NHS work must take priority – thank you for your understanding.
